This is a home-office based, prevention and education focused opportunity that will take you out of the mundane office setting and directly involved with seniors in designated communities and local travel in Rhode Island (RI).
Our select NPs are needed to conduct in-home, Annual Health and Wellness Assessments/Visits with our senior Medicare population. . As part of the health insurance benefits, medical evaluations are conducted face-to-face at the member's home. The visit is an opportunity for the clinician to reinforce the importance of staying current on covered preventive services and identifying potential gaps in care which may have stemmed from unnecessary hospitalization and ER visit.

No prescribing or treatment responsibilities - simply perform consultative assessments/evaluations.
Fixed payment per completed and submitted appointments.

Travel & mileage reimbursement (IRS standard rates for current year).
Pre-scheduled appointment schedules with Medicare members within your designated location and time frames.
CCMG will provide:
iPad tablet and/or laptop.
All needed medical supplies and/or equipment to accomplish designated tasks/assignments.
Credentialing services and paid training.
Liability/Malpractice insurance.
Requirements:
Current state licensed and board-certified NP in Rhode Island, with NPI.
6-months of experience in conducting wellness exams preferred.
Master's Degree in Nursing (preferred).
1-year experience in Medicare Advantage (MA) in-home risk assessment preferred.
EMR skills (preferred).
Knowledge of HCC, RAF, and HEDIS (preferred).
Bilingual preferred - English/Spanish.
Travel locally and to surrounding areas, 80% to 90% of your scheduled availability.
Dedicated, safe and private work space and existing Wi-Fi when working from home.
