Tabitha Home Health Care and Hospice, LLC - A Collaboration with Immanuel in Omaha, NE is seeking a Social Worker to work PRN/as needed. This schedule includes a weekend and holiday rotation.
The Social Worker is responsible for providing direct social services to Seniors/residents/clients and family members to ensure that the medically related emotional and social needs of the Senior/client are met or maintained. This position is also responsible for needs assessment, care planning, problem resolution, discharge planning and client advocacy, and completion of documentation per State and Federal legislations. The Social Worker embraces all components of the job, fostering an environment that allows Tabitha to provide optimum care to Tabitha’s Seniors/clients.
Essential Functions
Ideal Candidate
EDUCATION: Bachelor’s degree from an accredited school of social work, required (casework to be supervised by a MSW). Graduate Masters from an accredited school of social work, preferred.
EXPERIENCE: One year of clinical experience appropriate to the counseling and casework needs of the terminally ill and/or medically ill required. Home Health Care, Nursing Home or Hospice experience preferred. Training and experience in dealing with grief preferred. Basic knowledge of health care funding sources, including Medicare, Medicaid, and insurance required.
CREDENTIALS : Must be certified by the State of Nebraska as a bachelor’s or master’s level social worker prior to employment. CPR required. License must be kept up to date at the expense of the employee. CPR Certification must be kept up to date at the expense of the employee. Valid Nebraska Driver’s license (unless otherwise allowed by law) and a good driving record required. Must possess auto insurance at or above Nebraska statutory requirements. Must meet Tabitha’s vaccination requirements.
Compensation
At Eventide | Tabitha, we value our employees and offer a competitive starting pay range of $24.04-30 per hour for Social Worker's based on qualifications, experience, and location. Shift premiums and differentials are also available, for select positions, paid on employment status and shift worked.
